Accident Investigation with DATA LOGGER REPORTS DIGITAL REPORT EXCEPTION REPORT TRACK SIMULATION RECORDING ACCIDENT REPORTS OF DATA LOGGER • SYSNCHONISATION OF CLOCK • CORRECT FUNCTIONING OF DATA LOGGER • HEALTHY DATALOGGR NETWORK • SELECTION OF SPECIFIC INPUTS • SELECTION OF EXACT TIME INTERVAL • CORRECT ANALYSIS OF REPORT • PRESENTATION OF REPORTS • STORING OF REPORTS Case – I Derailment of the Rake of Garib-Rath Express Derailment of the Rake of Garib-Rath Express at BANDRA TERMINUS on 01.05.2014 Brief Description of incident • Rake of Bhava Nagar Exp was standing on P.F-2 • Rake of Garib Rath Exp was standing on P.F-3 • SM/BDTS cleared Sh -47 for moving the Bhavanagar Exp rake from PF -2 • But he wrongly announced to start the rake of Garib rath Exp from PF-3 • The Pilot of Garib rath EXP followed the instruction from SM and with out observing the signal proceeded . • As a rsult the rake of Garib rath derailed on Point No 141 & 143 in facing direction damaging both the points Brief Description of incident • Noticing his fault SM/BDTS hurriedly cancelled the SH – 47 which was cleared earlier. • Movement from PF -3 requires clearance of SH -43 • To clear SH -43 Point No 141 ,149 & 143 are to be locked in normal setting Observations before derailment • The rake of Garib Rath was standing on PF-3. • On PF-2 rake of Bhavnagar Express was standing. • Both the trains had just arrived half an hour back. G rath Bhavanagar Physical Observations after the incident • Both the tounge rails of Point 141 had moved away from their orignal positions. • One of the wheel was between tounge rail and the stock rail. • The ground connections were intact but the rods were bent. • Point Machine was not damaged. Rollers seem to be O.K. • Point 143 only one of the rod was bent. The wheel had not derailed on this point.
