Kind attention is invited to Railway Board’s letter cited under reference wherein
policy guidelines on reimbursement of cancer treatment taken in emergency have been
issued to zonal railways.

Zonal Railways have sought clarification from Railway Board w.r.t. treatment on
first occasion as mentioned in the letter cited under reference - “It is however clarified that
reimbursement of treatment on first occasion in any case shall not be construed as referral
for undertaking future/follow up treatment for the cancer ailment and subsequent treatment
shall be undertaken after obtaining referral from railway medical authorities.”

The matter has been examined in consultation with Finance Directorate of Railway
Board and it has been decided that treatment on first occasion shall mean "First cycle of
Chemotherapy or first cycle of Radiotherapy or surgery, if prescribed by the treating
doctor and undergone initially after diagnosis of cancer, may be treated as the ‘first
occasion of treatment' and any subsequent cycles/doses of chemotherapy or
radiotherapy or any subsequent surgery do not fall under the "first occasion of
treatment" and will require permission from authorities concerned."

This has the approval of Board (MF and CRB & CEO).
